Es sabido que el<strong> exceso de macronutrientes en los tejidos adiposos<\/strong> <strong>estimula<\/strong> a los mismos <strong>a liberar mediadores inflamatorios<\/strong> y<strong> a disminuir la secreci\u00f3n de inhibidores de la inflamaci\u00f3n<\/strong>, <strong>creando<\/strong> un<strong> ambiente proinflamatorio<\/strong> y de <strong>estr\u00e9s oxidativo<\/strong>.<\/p>\n<p>La <strong>obesidad<\/strong> es uno de los<strong> elementos caracter\u00edsticos del s\u00edndrome metab\u00f3lico<\/strong>. Parece que la <strong>relaci\u00f3n entre estos dos factores<\/strong> se establece por medio de la <strong>inflamaci\u00f3n<\/strong>.<\/p>\n<p>Existen <strong>dos tipos de tejido adiposo<\/strong>: <strong>pardo<\/strong> y <strong>blanco<\/strong>. En reci\u00e9n nacidos, el<strong> tejido adiposo pardo<\/strong> ayuda a regular el gasto energ\u00e9tico mediante termog\u00e9nesis.<strong> En adultos<\/strong>, <strong>existe una correlaci\u00f3n inversa entre el tejido adiposo pardo y el \u00cdndice de masa corporal<\/strong> (IMC). Esto es, <strong>a mayor IMC<\/strong>, <strong>menor cantidad de tejido adiposo pardo<\/strong>. Esto se aprecia especialmente en personas mayores, lo cual hace pensar en el papel de este tejido en el metabolismo humano. El <strong>tejido adiposo blanco<\/strong> se pensaba que era una especie de tejido inerte, sin otra funci\u00f3n que el almacenaje de energ\u00eda. Ahora sabemos que su papel es mucho mayor, y que <strong>participa activamente en la regulaci\u00f3n de procesos tanto fisiol\u00f3gicos como patol\u00f3gicos<\/strong>, <strong>incluyendo<\/strong> <strong>inmunidad<\/strong> e <strong>inflamaci\u00f3n<\/strong>.<\/p>\n<p>La <strong>inflamaci\u00f3n<\/strong> es una secuencia ordenada de eventos <strong>consistente en la liberaci\u00f3n de mediadores<\/strong> y <strong>expresi\u00f3n de receptores qu\u00edmicos en nuestro organismo<\/strong>, con el <strong>objetivo de mantener la homeostasis de los \u00f3rganos y tejidos<\/strong>, y tambi\u00e9n es un <strong>mecanismo de respuesta al da\u00f1o<\/strong> o <strong>destrucci\u00f3n de los tejidos<\/strong>.<\/p>\n<p>Existen <strong>dos tipos de inflamaci\u00f3n dependiendo de su duraci\u00f3n<\/strong>: <strong>aguda<\/strong> y <strong>cr\u00f3nica<\/strong>. Las diferencias entre estos dos tipos son:<\/p>\n<ul>\n<li>En la <strong>inflamaci\u00f3n aguda<\/strong>, la <strong>duraci\u00f3n es corta<\/strong>, y a <strong>nivel celular se caracteriza<\/strong> por <strong>edema<\/strong> y <strong>proliferaci\u00f3n de leucocitos<\/strong>.<\/li>\n<li>La <strong>inflamaci\u00f3n cr\u00f3nica<\/strong> tiene <strong>mayor duraci\u00f3<\/strong>n, y en ella se <strong>producen fen\u00f3menos de proliferaci\u00f3n de vasos sangu\u00edneos<\/strong> <strong>y tejido conectivo<\/strong>,<strong> adem\u00e1s de migraci\u00f3n<\/strong> de otras c\u00e9lulas del sistema inmune: los <strong>linfocitos<\/strong> y los <strong>macr\u00f3fagos<\/strong>.<\/li>\n<\/ul>\n<p><strong>Estos fen\u00f3menos inflamatorios<\/strong> est\u00e1n <strong>mediados por diferentes<\/strong> y <strong>numerosas mol\u00e9culas<\/strong>, como la Interleukina-<strong>6 (IL-6)<\/strong>, el factor de necrosis tumoral-\u03b1 (TNF-\u03b1), o la prote\u00edna C reactiva (PCR).<\/p>\n<p>La <strong>obesidad se ha relacionado ampliamente con un estado inflamatorio cr\u00f3nico<\/strong>. Este estado es un<strong> factor de riesgo cardiovascular<\/strong>, es el sustrato de enfermedades como trastornos de coagulaci\u00f3n, arterioesclerosis, resistencia a insulina y diabetes.<\/p>\n<p>Cuando <strong>existe un estado de exceso de nutrientes<\/strong>, se<strong> producen m\u00e1s c\u00e9lulas adiposas<\/strong> (hiperplasia) y <strong>de mayor tama\u00f1o<\/strong> (hipertrofia). Esto <strong>hace m\u00e1s probable que los vasos sangu\u00edneos no lleven ox\u00edgeno suficiente al tejido adipo<\/strong>so (hipoxia). Esto<strong> produce cambios en el tejido<\/strong>, como <strong>muerte celular<\/strong> (necrosis), lo cual es <strong>interpretado como un da\u00f1o<\/strong> generando secreci\u00f3n de las sustancias proinflamatorias descritas anteriormente, as\u00ed como radicales libres.<\/p>\n<p><strong>Numerosos estudios describen una relaci\u00f3n positiva entre los niveles de obesidad y IL-6.<\/strong> De hecho, un tercio de las concentraciones de IL-6 que circulan en nuestra sangre se originan en el tejido adiposo. <strong>Otro marcador muy correlacionado con obesidad<\/strong> es<strong> la prote\u00edna C<\/strong> reactiva, la cual aumenta cuando lo hace el \u00edndice de masa corporal.<\/p>\n<p><strong>Niveles elevados de PCR<\/strong> se <strong>relacionan con infarto de miocardio<\/strong>, hipertensi\u00f3n, <strong>eventos cardiovasculares en personas diab\u00e9ticas<\/strong>, e ictus. La<strong> PCR se sintetiza en el h\u00edgado<\/strong> y esta s\u00edntesis es <strong>mediada por IL-6<\/strong>.<\/p>\n<p>Esta <strong>sobreexpresi\u00f3n de sustancias proinflamatorias<\/strong> se<strong> considera el nexo de uni\u00f3n<\/strong> entre <strong>obesidad<\/strong> e <strong>inflamaci\u00f3n<\/strong>. El <strong>aumento de mediadores proinflamatorios<\/strong> se <strong>relaciona con riesgo cardiometab\u00f3lico<\/strong>, arterioesclerosis, <strong>enfermedad coronaria<\/strong> e insuficiencia card\u00edaca.<\/p>\n<p>La <strong>adiponectina es una sustancia que tiene un efecto antiinflamatorio<\/strong> y<strong> anti aterog\u00e9nico<\/strong>. Precisamente es <strong>producida por los adipocitos<\/strong>. Sin embargo, <strong>su liberaci\u00f3n disminuye cuando aumenta el tejido adiposo<\/strong>. Los niveles altos de PCR, IL-6 y radicales libres se asocian a reducci\u00f3n de adiponectina. De hecho, la administraci\u00f3n de agentes antioxidantes reestablece la producci\u00f3n de adiponectina, protegiendo contra el da\u00f1o del endotelio vascular producido por el estr\u00e9s oxidativo.<\/p>\n<p>Est\u00e1 c<strong>omprobado que la restricci\u00f3n cal\u00f3rica<\/strong> y la<strong> p\u00e9rdida de peso subsecuente<\/strong>, <strong>reducen los niveles de estos marcadores proinflamatorios<\/strong>. En cuanto al tipo de dieta, <strong>dietas con alto contenido en az\u00facar<\/strong> han mostrado <strong>incremento de estos marcadores sist\u00e9micos de inflamaci\u00f3n<\/strong>. En un estudio con sujetos adultos sanos que tomaron alimentos ricos en glucosa o fructosa durante dos semanas, se evidenci\u00f3 un aumento de la PCR. Las <strong>grasas trans en la dieta<\/strong>,<strong> aumentan tambi\u00e9n los niveles de sustancias proinflamatorias<\/strong>. Por el contrario, <strong>dietas ricas en fibra<\/strong>, ya sea en la propia dieta o como suplemento, han demostrado una r<strong>educci\u00f3n en los niveles de PCR y IL-6<\/strong>.<\/p>\n<p>Todos estos datos demuestran que el <strong>exceso de peso es equivalente de un estado proinflamatorio que provoca estr\u00e9s oxidativo y da\u00f1a el endotelio de nuestras arterias<\/strong>, y que podemos r<strong>evertir este estado por medio de una dieta que restaure nuestro peso ideal<\/strong>, y que adem\u00e1s sea <strong>rica en agentes antioxidantes y antiinflamatorios<\/strong>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>La obesidad es una acumulaci\u00f3n de grasa en exceso que puede interferir con un estado de salud \u00f3ptimo.
